Reporting a Conflict of Interest

Please provide, at a minimum, the following information to report a possible conflict of interest:

  • the name of the person(s) with the conflict of interest
  • the nature of the conflict of interest
  • any evidence or information you have to support the report
  • (if relevant) any other person who may have additional information to support the report.  

This form provides you with the opportunity to anonymously report your concerns about possible conflicts of interest in the workplace. You must provide sufficient information to enable the department to undertake enquiries on your report.
